this document contains the health systems plan (hsp) and the annual implementation plan (aip) for residents of michigan's health service area six. three sections constitute the hsp: (1) introduction--authority of the health systems agency, agency mission and purpose, purpose and use of the hsp, the plan development process, evaluation of the plan, and scope and limitations of the plan; (2) description of health service area--geography and transportation, historical characteristics, population characteristics, socioeconomic characteristics, and general health indicators (mortality, morbidity, disability, occupational exposure factors, environmental exposure factors, community health promotion and protection, prevention and detection, diagnosis and treatment, habilitation and rehabilitation, maintenance, support services, and enabling services); and (3) health goals--priority setting process, state and federal priorities, the impact of the health system on health status, and goals related to health status and health system indicators noted in the preceding section. areas of future development are identified as they relate to health status and health services, and state and federal goals not incorporated in the hsp are examined. the aip provides information on the authority of the health systems agnecy, the purpose and use of the aip, the planning methodology, the prioritization process, and the time frame for development. objectives, recommended actions, and resource requirements are specified for health status and health service areas covered in the hsp
